Form Field and Question Types

In this article, we'll describe the type of questions/fields that can be utilised in Acorn forms. Forms are used in Feedback form activities, Live Learning session registration and Momentum Workflows.

Text: Single Line

This field allows a user to provide an answer in a single line of text. This should be utilised for fields such as names, titles or other short details.

Text: Multi-line

This field allows a user to provide an answer requiring multiple lines of text. This could be utilised for extended response answers such as feedback or comments.

File Upload

This field allows users to submit files. You can limit the number of files a user can upload from 1 - 5.  Alternatively, you can allow user to upload an unlimited amount of files. You can also limit the size of the files that users are permitted to upload. There are no restrictions on the types of files that users can upload.

Date Picker

This field allows users to select a date from a calendar. You can also require users to input a time if required.

Duration

This field allows users to specify an amount of time in Years, Months, Weeks, Days, Hours, Minutes and Seconds.

Cost

This field allows users to insert a numerical value to indicate a specific cost or price.

Percentage

This field allows the user to specify a percentage between 1-100.

Rating

This field allows users to provide a rating. The scale, shape and colour can be configured.

Ranking

This field allows users to rank a number of options. Users can click and drag options to arrange the order or use the up and down arrows.

Radio Option

This field allows users to select a single option from a list of known potential options. Options can be listed vertically or horizontally.

Checklist

This field allows users to check a box against a list of options.

True or False

This field allows users to decide if a statement is true of false. 

Numerical

This field allows users to submit a numerical value against a statement or question.

Likert Scale

This field is used to gauge a users level of agreement or disagreement with a statement, typically offering a range of response options from strongly agree to strongly disagree.

CPD Points

This field allows users to enter a numeric value against each of the CPD categories that apply to their user account. Please note - this does not award users CPD points.

User Selector

This field allows users to select from a drop down list of users. The list that is displayed can be restricted to All Users, Selected Users or Selected Cohorts.

Matching

This field allows users to click and drag an option to align with a statement or item.

Instructions

This field, previously known as Content, enables administrators to provide instructional text to users when viewing a form.

Signature

This field allows users to apply a signature. Users can draw or type their signature.
