How to Create a Feedback Form Activity

Creating a Form Activity

In this article we will show you how to create a basic Feedback Form activity. Feedback forms can be found in either a Course, or Live Learnings. In this example we'll go over how to add a feedback form to a Course.

 

1. Proceed to a Course > Click Screenshot 2023-09-01 at 2.09.33 PM.png  > Start Editing > Create Activity Screenshot 2023-09-12 at 10.19.38 AM.png .

2. Select 'Feedback Form' from the Activity Type dropdown menu.

 

3. Fill in the following details:

  • Feedback Name - The name that the feedback form will display on the course page.
  • Feedback Description - Describe the feedback form you are creating. 
  • Show Description - Toggle this off if you wish to hide your description
  • Edit Form - This is where you will find your feedback form 
  • View Submissions - Clicking this will  redirect you to a Form Report for you to view all the signature submissions
  • Anonymous - Toggle this on if you wish to make the form anonymous.
  • Email To Completed Users - When a user completes this course, an email will automatically be sent to them with a link to this feedback form. 
  • Hidden To Users - Toggle this on to hide the feedback form from your users
  • Course Completion Prerequisite - Choose when the form will be available to your users. 'Always available' is the default choice.
  • Completion Criteria - Set the completion criteria so your users, must complete the feedback form, mark themselves complete, or disable the activity completion to make the signature optional.

 

4. Click Screenshot 2023-11-20 at 11.49.29 AM.png > Configure your Feedback Form.

  • Star Rating  
    • Label - Title of the rating. Let the user know what they are rating. Ex. Rate the Course.
    • Description - Describe what the rating is intended for. Ex. Give a rating out of 5 for the course. (Optional)
    • Help Text - Provide some instructional text for your user that will be displayed in a help bubble. (Optional)
  • Text (Multi Line) 
    • Label - Title of the text. Let the user know what they are providing feedback on. Ex. What did you think of this course?
    • Description - Describe what your user should add to their feedback. Ex. Give an overview of what you thought of the course. (Optional)
    • Help Text - Provide some instructional text for your user that will be displayed in a help bubble. (Optional)
  • Radio - Gives the ability to answer a multiple choice like question 
    • Label - Title of the Radio Option. Ex. Did you like this course? Yes or No. 
    • Description - Describe the question that you are asking your user. (Optional)
    • Help Text - Provide some instructional text for your user that will be displayed in a help bubble. (Optional)
    • Options - Give your users some options to choose from. Ex. Did you like this course, Yes or No? You can add as many options as you need.
