Creating a Feedback Form Template

Feedback forms are an essential tool in educational and training environments, enabling instructors and organizations to gather valuable insights from learners about their experiences. With the recent updates to our PLMS, feedback forms have transitioned from the Activity Bank to Form Templates, providing a more streamlined and efficient process for creating and deploying feedback mechanisms across courses or live learning sessions. This article outlines the process of creating a feedback form within the Form Templates, its importance, and the steps to integrate it into your courses.

 

To create a Feedback Form in Form Template, follow these steps:

  1. Navigate to the Form Templates
  2. Create a New Feedback Form
  3. Design Your Form
  4. Add the Form Template to a Course

 

Navigate to the Form Templates

1. Proceed to Admin > Tenancy Management > Form Templates.

Create a New Feedback Form

  • Template Name - Title of the form. Ex. Feedback Form.
  • Template Description - Description of the Form. Ex. Feedback.
  • Form Type - Feedback activity form.

Design your Form

  • Use the form builder to add questions and fields relevant to the feedback you wish to collect. You can include a variety of question types.
  • Once you're satisfied with the form, save it as a template for future use across multiple courses or sessions.

2. Find the New Feedback Form > Click Screenshot 2024-02-07 at 11.59.06 AM.png > Click 'Publish'.

  • Clicking 'actions' also gives you the ability to copy, edit, delete, or deactivate the Feedback Form Template.

 

  • Star Rating  
    • Label - Title of the rating. Let the user know what they are rating. Ex. Rate the Course.
    • Description - Describe what the rating is intended for. Ex. Give a rating out of 5 for the course. (Optional)
    • Help Text - Provide some instructional text for your user that will be displayed in a help bubble. (Optional)
  • Text (Multi Line) 
    • Label - Title of the text. Let the user know what they are providing feedback on. Ex. What did you think of this course?
    • Description - Describe what your user should add to their feedback. Ex. Give an overview of what you thought of the course. (Optional)
    • Help Text - Provide some instructional text for your user that will be displayed in a help bubble. (Optional)
  • Radio - Gives the ability to answer a multiple choice like question 
    • Label - Title of the Radio Option. Ex. Did you like this course? Yes or No. 
    • Description - Describe the question that you are asking your user. (Optional)
    • Help Text - Provide some instructional text for your user that will be displayed in a help bubble. (Optional)
    • Options - Give your users some options to choose from. Ex. Did you like this course, Yes or No? You can add as many options as you need.

Note: These are just a few examples of the types of questions you can ask using the form.

Adding the Form Template to a Course

  • After creating your feedback form template, you can easily add it to any course or live learning.
  • In the course settings, look for the 'Template' dropdown menu at the course activity level and select your newly created feedback form template.

2. Click 'Create Activity' > From the 'Activity Type' dropdown > Select 'Feedback Form'.

 

3. Fill in the following details:

  • Feedback Name - The name that the feedback form will display on the course page.
  • Feedback Description - Describe the feedback Form you are creating. 
  • Show Description - Toggle this off if you wish to hide your description
  • *Template* - From the Template dropdown, select your Feedback Form. 
  • Anonymous - Toggle this on if you wish to make the feedback form anonymous.
  • Email To Completed Users - When a user completes this course, an email will automatically be sent to them with a link to this feedback form. 
  • Hidden To Users - Toggle this on to hide the feedback form from your users
  • Course Completion Prerequisite - choose when the form will be available for your users. 'Always available' is the default choice.
  • Activity Prerequisites - Activity prerequisites allow you to define which other activities must be completed before this activity can be accessed by your users.
  • Completion Criteria - Set the completion criteria so your users, must complete the feedback form, mark themselves complete, or disable the activity completion to make the signature optional.
